Health Services

Pre-School and Kindergarten students are required to submit the following paperwork to the Health Office prior to the start of school:   

 

** Immunization records (must meet NJ State requirements with booster shots). 

** Physical exam form completed by your child's doctor (within 365 days of the start of kindergarten)   

** Growth & Development form completed by parent

** Dental exam form completed by child's dentist (within 365 days of the start of kindergarten).

PRE-SCHOOL STUDENTS are required to have a flu shot between September 1st and December 31st each year.

As per N.J.A.C. 8:57-4.19, children six months through 59 months of age attending any licensed child care center, or preschool facility on or after September 1, 2008, shall annually receive at least one dose of influenza vaccine between September 1 and December 31 of each year.

• Please check with your child's doctor to make sure your child's immunizations meet NJ State requirements so they will be able to start on the first day of school. 

• Parents with children who have a September birthday should contact their child's doctor about a immunization visit for just booster shots prior to the start of school. Those children with a September birthday can receive their booster shots prior to the start of school, then can have their well check up in September. If your student has their physical exam or immunization appointment after the first day of school, the nurse needs written documentation from your child's doctor with the appointment date prior to the start of school. 
• Please inform the nurses of any severe allergies or medical conditions prior to your child starting school to ensure the proper forms are completed and medication is in school.  No child may transport medication to school, only an adult may bring in medication.    

 

During the summer, email is the best way to reach the health office. Please allow up to 2 weeks  for a response when school is not in session during late June, July and August.  

Requirements for students transferring into district:

• DPT Vaccine Series (must have a minimum of 5 doses with the latest one given on or after 4th   birthday)
• Polio Vaccine Series (must have  a minimum of 4 doses with the latest one given on or after 4th birthday)
• Measles, Mumps and Rubella  (must have two doses- one after one year of age & one prior to kindergarten). Written proof of a positive MMR titer is accepted in lieu of MMR #2. 
• Hepatitis B Vaccine (completed series-usually 3 doses) or written proof of titer
• Varicella (Chicken Pox) Vaccine or Documentation of Disease
• Physical examination form must with completed by your child's doctor with 30 days of enrollment.  then the student if have a physical exam within 30 days.  Written proof of physical exam within 365 days prior to enrollment this may be submitted to the health office for review.
• Growth and development form must be completed by parents/guardian within 30 days of enrollment.  

Preschool Requirements:

• All preschoolers are required to receive the previously listed vaccinations/exams with consideration to the age of the student.
• One dose of HIB vaccination is required for preschool entrance. 
• One dose of pneumococcal conjugate vaccine.
• Yearly requirement  - One dose of influenza vaccine given between September 1st and  December 1

Kindergarten Requirements:

• DPT Vaccine Series (4 doses, with one dose given on or after the 4th birthday, or any 5 doses)

Medication Administration

* All medications must be kept in the nurse's office. Students are NOT permitted to carry any medications at any time.  This includes cough drops, cold medicine, aspirin, vitamins, creams and any over-the-counter medications. 

* If your child requires ANY medication during the school day, you MUST have the proper medication administration order form filled out by his/her physician and signed by the parent/guardian. 

* Medication forms can be found under the "Health Office Forms" tab.

* The medication must be kept in the original bottle/container with the pharmacy label on it be accompanied by written permission and directions for the school.  Medication must be brought into the school by an adult

* Your child's medication that is in the health office will need to be picked up at the end of the year. 

* New medication orders will need to be written for the following school year and new medication will need to be dropped off. 

Children with reactive airway disease/asthma, who have medication in school, are required to have an NJ state mandated Asthma Action Plan.  The Asthma Action Plan  is completed by your child’s physician and signed by the parent/guardian.  Please make sure that both sides of the form are filled out completely and returned to us as soon as possible. The information in the Asthma Treatment Plan enables us to assist your child should there be a respiratory emergency during school hours.

Food, Medication and other Allergies

If your child has any allergies you must contact the health office prior to your child starting school.

If your child has life-threatening allergies and requires the use of an Epi-pen, please contact the health office ASAP so we can provide a safe learning environment for your child. 

If your child has been prescribed an Epi-pen for their allergy please have their physician completely fill out the Epi-pen administration order form located under the "Health Office Forms" tab. We will need to have their Epi-Pen in the health office prior to the start of school.

The Epi-pen must be brought into the school by an adult. It must be brought in the original packaging with the pharmacy label on it.  No child may transport medication to school, only an adult may carry medication to school.

Attendance

When your child is absent, it is the parent's responsibility to call the Office of Student Services at 298-2600 ext. 4134  by 9:00am. Please leave a voice mail with your child's name, grade, teacher and nature of illness, injury or reason for absence. 

Casts, Crutches, and Braces

Students with crutches, casts or other medical conditions that may be aggravated by  physical activity; running, jumping, kicking etc. will be excluded from recess until such time a physician’s note is presented allowing them to resume normal activities. The same applies to physical education classes.

Criteria to Return to School after Illness

Please contact the health office for when your child can return to school after being sick. During this time, we use  guidelines from the Burlington County Health Department along with our district policies to determine their return to school date. If your child exhibits any GI symptoms or fever, they must remain out of school until free of those symptoms for 24 hours.

Get Eye Smart  -  The American Academy of Ophthalmology.  Gives parents some information on pink eye. The BRSD requires any student suspected of having conjunctivitis to be seen by their doctor. Since conjunctivitis is very contagious having the correct diagnosis from your child's doctor is very important. All student's returning to school after a suspected case of conjunctivitis must present a doctor's note.
