Welcome to the Health Office Page

My name is Lucy Carney and I am the school nurse at Peter Muschal Elementary School. On this page, you’ll find lots of important information to help keep your children healthy and safe. I look forward to working with you throughout the school year!

Some Important Information

Attendance
If your child will be late or absent, please remember to fill out the Report Absence/Tardy Google Form located on the Peter Muschal Main Page, or call Mrs. Romano in the Main Office at 609-298-2600 ext 4100.

Contact Information
Please ensure your contact numbers are up to date. It is essential that we can reach you in the event your child becomes sick or injured during the school day.

Medication Administration

If your child needs to take any medication during the school day, you must submit a completed Medication Administration Order Form from their physician. This includes both prescription and over-the-counter medications.

  • Students are not permitted to carry any medication with them during school hours.

  • All BRSD schools may administer Tylenol and Ibuprofen to students with parental consent through InfoSnap, so these medications should be kept at home.

  • The medication form must be completed by the physician and signed by a parent/guardian.

  • All medication must be brought to school by an adult.

  • Prescription medications must be in the original bottle or packaging with the pharmacy label attached.

Students with Asthma

If your child has asthma and will be using an inhaler at school, a New Jersey State-Mandated Asthma Action Plan must be completed.

  • The form is located under the Health Office Forms tab.

  • Please ensure both sides of the form are completed by both the parent/guardian and physician.

  • Return the form to the health office as soon as possible.

Students with Life-Threatening Allergies

If your child has a life-threatening allergy that requires an EpiPen:

  • Please contact the health office so we can ensure a safe learning environment.

  • An EpiPen Administration Order Form must be completed by the prescribing physician.

  • The form is located under the Health Office Forms tab.

Reminder:
All medication administration forms are valid for one school year only. Forms must be renewed and resubmitted each school year.
