Pre K and Pre School Enrollment Requirements

PreK students need the following requirements:

As per N.J.A.C. 8:57-4.19, children six months through 59 months of age attending any licensed child care center, or preschool facility on or after September 1, 2008, shall annually receive at least one dose of influenza vaccine between September 1 and December 31 of each year. 

Most PreK students will need booster shots to ensure that they are fully immunized.  When your child goes for their physical exam please check with the doctor to see if they have received their booster shots.

Please note that when your child starts kindergarten they will need to have new a Physical Exam Form, Growth & Development Form, and Dental Form completed with updated immunizations prior to the start of kindergarten.  

** Immunization records

** Physical Exam Form completed by your child's doctor 

    (within 365 days of the start of PreK)

** Growth and Development Form completed by the parent

** Dental Form completed by child's dentist 

    (within 365 days of the start of PreK)

       All forms are located below and may be downloaded.  

      ** Please inform the nurses of any severe allergies or medical conditions prior to your child starting school to ensure the proper forms are completed and medication is in school.  No child may transport medication to school, only an adult may bring in medication.

During the summer email is the best way to reach the health office. Please allow up to 2 weeks for a response when school in not in session during late June, July and August.
